Diabetes Education

Diabetes education services at Boone Hospital offers outpatient medical nutrition therapy counseling and support to patients with prediabetes, Type I diabetes, Type II diabetes, and gestational diabetes. Our diabetes education program is accredited by the American Diabetes Association.  Our team consists of Certified Diabetes Educators (CDE), which includes a Registered Nurse (RN), and Registered Dietitians (RD).   

Our outpatient diabetes education provides:

  • Diabetes Self-Management Program
  • Comprehensive nutrition counseling

Diabetes Self-Management Education Program

Boone Hospital Center’s Diabetes Self-Management Program consists of individual and group classes. Our program meets national standards, includes high-quality education and is recognized by the American Diabetes Association. Our team includes registered licensed dietitians and certified diabetes educators. Monthly Interactive Diabetes Self-Management Education classes consist of a series of four classes, each with specific content topics.  We use educational tools recommended by the American Diabetes Association and visuals that help with learning information.  We empower each person to utilize tools from each class that will work for them. One-on-one instruction is available for those who have special needs and cannot attend a group class.

Our Process

Initial Assessment

Initial one-on-one appointment with RD or CDE.

Fundamentals Classes:

When: A series of classes that meets 2 hours a week for 4 weeks on Tuesdays from 4:30-6:30 PM

This course is ideal for those who have attended the intro and Advanced class and are looking to improve the management of their diabetes.

Our team of registered dietitians, registered nurse, and certified diabetes educators can work with you on short- and long-term plans. What should I expect?

Generally, our initial visit lasts about an hour.  A follow-up Medical Nutrition Therapy visit typically lasts about 30 minutes, but may be longer or shorter depending on your needs. We also offer diabetes education classes. This series of four classes last 2 hours each and meet once a week for a month. Learning to manage diabetes can be challenging; diabetes is a complex and serious disease.  Though diabetes is a common disease, every individual needs unique care. Our team of experts works with you and your loved ones, whether in an individual or group class, to help you learn as much as possible about key areas of diabetes care. We work with you to help set priorities and goals, coach you in these areas and empower you to feel better able to respond to your changing needs.
