Virtual Visits

Diabetes and nutrition counseling are now available through virtual visits!

To set up a virtual visit with a Boone Hospital Center dietitian:

1. Complete and submit the appropriate new patient form:
   1. Diabetes
   2. Gestational Diabetes
   3. Nutrition
2. Download the Zoom Cloud Meeting app on your Apple or Android smartphone or computer.
3. A Boone Health representative will send you additional information via secure email or MyChart.
4. On the day of your appointment, a Boone Health will send you a private Zoom Meeting invitation.
