We suggest that you contact your health insurance company regarding coverage.   It is your responsibility to contact your insurance company regarding coverage or prior authorization for any outpatient Diabetes & Nutrition Counseling services requested by your physician.

Helpful Information
When you call your insurance company, you may be asked to provide the following information:

1. Diagnosis
2. Reason for referral
3. Provider
4. CPT Code (common CPT codes are 97802 and 97803)

Example: My diagnosis  is ____, I am calling to check on benefits of receiving nutrition education at Boone Hospital Center on an outpatient basis.  My CPT Code is 97802 (first individual visit for nutrition counseling (MNT)) or 97803 (follow-up individual visit for nutrition counseling (MNT)).
