Boone Health Connection is Boone Health’s patient portal. It provides you with confidential and secure access to your health information. From the convenience of your home or on your mobile device, you can access test results, your medical history, upcoming appointments, and more.
All the information contained in your profile on the patient portal is generated from your Boone Health electronic health record.
No, use of the Boone Health Connection patient portal is provided free of charge to any patient of Boone Health.
Patients who have visited a Boone Health location since April 1, 2021 may use the self-enrollment feature.
Upon patient registration, you will be asked if you would also like to register for the patient portal.

In addition to having access to your own health information, the Boone Health Connection patient portal allows you to grant access to others. Individuals at least 18 years of age can be given proxy access to your health records with an authorized consent.
Parents/guardians can apply for proxy access to health information for minor children (ages 0-11).
Parents/guardians of patients ages 12-17 may also gain proxy access to their adolescent children’s health information, with authorized consent from the patient.

Information in Boone Health Connection comes directly from your electronic health record. If you find errors, please ask your physician’s office to correct any inaccurate information at your next clinic visit.

Yes! The Meditech Health (or MHealth) app, available for Apple and Android devices, allows you access to Boone Health Connection on your phone, tablet or other smart device.
An email address and internet access are all that is required to use Boone Health Connection. The patient portal can be accessed at https://booneportal.meditech.cloud, or via the MHealth app on your smart device. Your logon ID must be linked to a unique email address; two users may not share the same email address.
All Boone services rendered before April 1, 2021 will continue to be accessible through the MyChart patient portal. Services rendered on or after April 1, 2021 will only be accessible through Boone Health Connection. Some basic data for services received prior to April 1, 2021 may appear in your Boone Health Connection portal. To access complete patient portal data prior to the switchover date, check your MyChart portal account.
The automated self-enrollment feature allows patients to enroll in Boone Health Connection with information that has been verified at a recent visit. If the system cannot match your submitted information to an account with 100% accuracy, the enrollment will fail. To protect our patients’ health information, enrollment requests that are not successfully matched are flagged for review by our Portal Team. An unsuccessful enrollment request does not preclude participation in the patient portal, only that a team member must review your information to correctly link your clinical account.

The first step is to make sure you are entering your logon ID and password correctly.
If you have previously accessed the portal, you should be using the Login Now link, instead of the Enroll Now link.
Make sure your browser is not auto-filling previous/temporary/incorrect logon information (refer to your browser settings and search “saved passwords”).
If you still cannot access the portal, your logon ID or password may have been mis-keyed upon enrollment. You can request a reset by clicking the “Forgot Logon ID?” or “Forgot Password?” links on the login screen. You may also email email@example.com or call us at 573-815-2750.
Boone Health Connection will automatically provide a messaging link to Boone providers that you have visited (physically or via tele-health), since the Boone Health go-live date of April 1, 2021. However, if you have not seen your provider on or after that date, the patient portal system does not recognize your relationship with your provider yet. Please contact the Boone Health Connection Portal Team for assistance in generating a messaging link for all your Boone Health providers.
