Please complete this form to have a copy of your medical records sent to you or to someone other than yourself. Note: Parents and guardians, please use this form for your patients.
For Third Parties
Authorization for Release of Information
Third parties, please complete this form to request a copy of an individual’s medical records. Note: The individual whose records are being requested must sign this authorization.

How long will it take to get a copy of my record?
The average turnaround time for a request is three (3) days. It depends on the format of the record and its overall availability. Boone Hospital Center maintains records in hard copy, digital format on CD and electronic archive and on microfilm. You may come to the office to pick up the record, but we encourage you to call 573.815.7010 to assure its availability.
Confidentiality
Boone Hospital Center Patient Bill of Rights assures each patient that his/her treatment is confidential and it will not be revealed to anyone without proper authorization. Since all Boone employees are involved directly or indirectly in patient care, confidentiality is an important issue for everyone. Breaches of confidentiality carry serious consequences. Please take a moment to review our HIPAA confidentiality policy.
May I look at my family's medical record?
Unless you are the patient's legal guardian, you may not review a family member's record without his/her signed consent.
Birth Certificates
Birth certificates are not available from Boone Hospital Center. To request a birth certificate please contact your local health department or the State of Missouri, Department of Health and Senior Services.
